Care Services Coordinator Greater Columbus Ohio

ALS United Ohio is a nonprofit organization seeking a social worker licensed in the state of Ohio who is interested in a fulfilling role working with people in Ohio with Amyotrophic Lateral Sclerosis (ALS). This is a full-time Care Services Coordinator position working in the Columbus office. Typical hours will be Monday through Friday 9-5, with monthly evening support group meetings, and occasional weekend fundraising events. Care Services Coordinator responsibilities include, but are not limited to, providing patients, families and caregivers with information about ALS, coaching individuals on disease management, connecting with local community resources, linking with local healthcare providers, facilitating a monthly in-person ALS support group and virtual support groups, a caregiver support group, and survivor support group. Other responsibilities may include helping manage an equipment loan program, providing adaptive equipment and other equipment and technology, assisting with a patient grant program, and working in partnership with other Care Services staff members locally and in other areas of the organization’s territory, collaborating with the Events and Development team to make connections related to potential corporate sponsors and individual donor prospects and identify grant opportunities and increasing outreach efforts to medical professionals, patients and families in an effort to not only increase the number of ALS patients registered with the organization but also to increase the services we provide.
