We are seeking a Registered Nurse to join our Home Health team. The ideal candidate will have experience providing direct patient care in a home health setting, including performing assessments, developing and implementing care plans, providing patient education, and collaborating with other healthcare professionals. The successful candidate will be compassionate and have excellent communication skills, as well as the ability to provide quality care in a fast-paced environment.
Responsibilities:
- Provide patient assessments and treatments in accordance with the plan of care.
- Provide patient education and discharge planning.
- Document patient progress and care in accordance with agency policies and procedures.
- Monitor vital signs, administer medications, and provide wound care as needed.
- Communicate with physicians, family members, and other healthcare providers to ensure continuity of care.
- Maintain patient confidentiality in accordance with HIPAA regulations.
