Alta Mira is a non-profit organization that has been supporting individuals with developmental or intellectual delays or disabilities, and their families, since 1985. We provide services to all age groups. We support over 800 children annually in our Early Intervention Program and 1500 adults in our Family Support Services. Position Specific Essential Job Functions
Supervisory Duties:
1. Schedules intake interviews with staff, as appropriate.
- Assures that Respite Home staff licensure, TB exams, medical certifications, CPR/First Aid, HWC and all other required training are up to date.
- Appropriately schedules staff and participants for the Respite Home.
- Assists and trains Respite Home staff to work directly with individuals who are dually diagnosed with behavioral problems and developmental disabilities.
- Available for on-call duty and/or substitute coverage for the Respite Home.
- Organizes, leads, and participates in Respite Home staff meetings.
- Reviews Respite Home staff time sheets and assures timely completion of calculations for data input.
- Assures that standards and regulations for licensing are maintained and monitors compliance.
9. Provides input into budgeting for Respite Home and adheres to budget guidelines.
10. Completes performance assessments for Respite Home direct care personnel in a timely manner.
- Consult with supervisor regarding any special circumstances, including emergencies.
Scheduling:
1. Takes initial referrals for Respite Home families.
- Complete pharmacist reviews quarterly.
- Seeks assistance from parents/guardians and on-call staff as needed.
- Works with Respite Home staff to delegate responsibility for individual(s) records as well as for the planning of individual and group activities geared to the specific needs of the weekend Respite Home attendees.
- Completes scheduling and staffing patterns for Respite Home staff in coordination with FSS Program Director.
- Coordinates Respite Home services with other FSSC’s and agencies.
- Coordinates and assists staff in designing and implementing appropriate services for Respite Home attendees.
- Ensure space is available utilizing the weekly schedule book.
- Assess appropriate staff to participant ratio based on needs of participants.
Attendance and Punctuality:
- Notification of absences 24-hours prior to shift
- Arrival to work within 5 minutes of scheduled shift
- Attendance at staff meetings/trainings (2x monthly)
- Inform supervisor of any changes in schedule prior to the change
- Submission of timesheets and Leave Requests according to agency policies and procedures
Participant Interactions:
- Consults with FSS Director to determine families and/or persons receiving services who need specialized services for residential or day placement, or entry into the system to get services.
- Assists with direct behavior management for specific persons receiving services to aid in their use of Respite Home, FSS and other community services.
- Brings specific problems or concerns to the FSS Director for discussion regarding long-term assistance.
Trainings:
- Orients and trains Respite Home staff.
- Attendance to Individual Specific Training (IST) for Respite Home participants.
- Completion of all Developmental Disabilities Supports Division training by given deadlines.
- Completion of agency annual trainings by given deadlines.
- Review of new participant files prior to working with the participant.
Documentation:
- Petty Cash verification form.
- Manages and assures appropriate utilization of Respite Home petty cash, including documentation and timely submission of required documentation.
- Monthly paperwork (Attendance Sheets, Petty Cash, fire drills) to be submitted to supervisor by the first day of the following month.
- Accurate records of attendance sheets as participants arrive and depart.
- Client medication documentation
- Incident Reports submitted within required timeframe
- Monthly Activity Calendar, weekly menu, and grocery list
- Progress Notes for each participant at each visit
- Development of weekly menu for each participant
- Mileage record – if needed
- Collection and accuracy of participant Physician’s Standing Order and Prescriptive Medication forms
- Petty Cash receipt form and receipts collected for activities.
Communication:
- Ensures “Crossover” discussion with co-worker from previous shift is done daily
- Communicates respectfully and professionally.
- Consults with supervisor regarding any concerns regarding participants/families
- Voice messages checked prior to participant’s arrival and upon return from activities
- Communication Log read prior to participants’ arrival
- Supervisor notification of any urgent/emergency situations
- Supervisor notification of any changes in staff schedule prior to the change
- Supervisor notification of any house or van maintenance needs
Household Duties:
- Complete weekly grocery shopping.
- Perform quarterly Costco shopping.
- Responsible for the cleanliness of the Respite Home.
- Develops monthly calendar of Respite Home activities and submits for newsletter publication on a timely basis.
- Coordinates with Respite Home staff and Facilities Specialist regarding maintenance of the Respite Home facility.
- Assures there are appropriate furnishings, equipment, and repair of the Respite Home facility in order to pass health and safety inspections.
- Car maintenance
- Other duties as assigned
