About Your Work
The Health Navigator assists members in navigating and connecting to clinical and community resources. Supports clinical staff in identifying and providing outreach, and baseline assessment services to members. Establish and maintain effective, ongoing relationships by facilitating communication and coordination with members, their caregivers, PCPs, and other providers. Connects members with health screening programs, and other resources. The Health Navigator will focus on care coordination and short-term case management for members needing support in accessing medical, social or behavioral services.
This position is located in various regions in California, and you must reside in one of the following areas with requirements to visit local hospitals : Los Angeles, Orange County, Sacramento, San Francisco Bay Area, or Santa Barbara
You will focus on :
- Under general supervision and guidance from the CM leadership team, the Navigator supports clinical staff through the completion of components of case management and disease management programs.
- Assist members in getting appointments & access to appropriate health care and community programs.
- Provide one to one guidance, support, education, coordination of care and other assistance to members as they move through the healthcare continuum.
- Initiate follow-up to confirm and coordinate additional needs.
- Participate in case conferences and meetings with the CM team and medical directors to support effective care coordination.
- Effectively establishes rapport with team members, patients / families, and physicians.
- Ability to provide a high level of customer service to members, internal and external customers in a professional, service-oriented, manner
- Educate and answer inquiries from members and or their caregivers about benefits, services, eligibility and referrals with a positive and professional approach, promoting member satisfaction and retention.

Physical Requirements : Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office / deskwork – Activity level : Sedentary, frequency most of workday.
