Job Overview

Th e Referral Coordinator is responsible for accurate and timely data entry of referral request into the appropriate portal . The Coordinator is also responsible for follow-up on all specialty referrals . The C oordinator is an integral part of the care team, working as a liaison between the patient, providers and Medical Management to ensure timely access to specialty care.

Minimum Requirements

• High school diploma required.

• Minimum of 4 years experience required.

• Medical Assistant certificate required.

• BLS certification required

• Bilingual English/Spanish required Bilingual English/Spanish required.
