AltaPointe’s Adult Residential Services are designed for consumers with serious and persistent mental illness who require social rehabilitation and community support. Combining medical, nursing and clinical services, our staff provides care and treatment for more than 200 adults in need of intensive service provision. The Adult Residential Services Program consists of group homes, which are supervised 24 hours/day when consumers are on the premises. Expected outcomes are the same for each adult group home to prevent re-hospitalization and to maximize and maintain successful community placement. Residential care focuses on the rehabilitation and return to the community of seriously mentally ill consumers.

Primary Job Functions

  • Supervises Adult Residential Program Operations
  • Coordinates and implements program activities for the residential group homes
  • Identifies problems, evaluates alternatives and implements effective solutions
  • Ensures that residential program standards are met
  • Reviews MARs and medication inventories to ensure appropriate medication management which includes timely refills and any necessary arrangements with medical staff for new prescriptions
  • Provides staff coverage and direct service duties as needed
  • Oversees the management of consumers’ appointments and ensures that appropriate transportation arrangements are made for the consumers
  • Completes necessary reports in a timely manner (i.e. vehicle and facility inspections)
  • Monitors effective hand off communication at shift change.
  • Takes calls from staff about emergencies or critical issues after normal work hours and on weekends as scheduled.
  • Assists with planning, coordinating, and facilitating Person Centered Planning meetings. Works collaboratively with support coordination, nurses, the individual, outside resources and their natural supports to achieve their desired goals.
  • Educates staff on the implementation of the Person Centered Plans.
  • Provides individuals the support and/or assist in accessing support needed to implement his Person Centered Plan and to achieve their personal outcomes.
  • Ensures assessments are scheduled and completed according to policies and that recommendations are followed. Complete quarterlies by their due dates.
  • Provides information and seeks guidance in the development and implementation of Positive Behavior Support Plans.
  • Participates in discharge planning when an individual leaves services.
  • Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation.
  • Provides supports to individuals that promote being present and participating in community life and inclusion.
  • Effectively demonstrates the application of competencies for adults with intellectual disabilities.
  • Facilitates building natural supports for individuals based upon their choices and


preferences.

  • Administers appropriate Human Resource functions
  • Provides training in the general orientation of the facility, policies and procedures, program operations and specific skill areas.
  • Is knowledgeable and applies Human Resource policies and procedures on a consistent basis.
  • Objectively completes employee performance evaluations on supervised staff and routes to Human Resources Department on a timely basis.
  • Provides staff with consultation and supervision, individually and through group staff meetings.
  • Ensures competence of staff to include ongoing compliance with Nurse Delegation Program and all AHS required training
  • Completes time and attendance within set timeframe given by payroll department
  • Manages staff schedules to ensure program staffing schedules are met.
  • Interviews and screens applicants when necessary and completes appropriate forms as outlined by Human Resources Policy and Procedures.
  • Objectively completes employee performance evaluations on supervised staff.
  • Provides positive and effective leadership.
  • Clinical Responsibilities
  • Demonstrates the ability to recognize the elements of a crisis state and takes a proactive approach to deescalate or resolve the situation
  • Effectively collaborates with the Person Centered Plan team members and outside sources in the delivery of clinical services
  • Orients consumer’s to the program upon admission and facilitates the coordination of care to a less restrictive environment
  • Clinical Record Keeping
  • Documents in a timely fashion per AltaPointe policy and program procedures
  • Completes all appropriate documentation for continuity of care
  • Documents in a clear and concise manner, services provided, individualized consumer response and progress towards treatment goals.
  • Ensures that all ALDOT reports are completed on program vehicles.
  • Documents incidents in RL6.
  • Supervision and Consultation
  • Seeks supervision and consultation as needed.
  • Accepts and employs suggestions for improvement.

Physical Requirements

  • Sitting occasionally to do documentation and/or ordering of supplies however, extended sitting could be up to 2 hours for documentation, and sitting can be in frequent bursts of up to 10 minutes.
  • Walking various distances on grounds can be frequent and up to a distance of 300 feet several times a week.
  • Simple grasp and/or pinch grip is needed to frequently handle smaller weighted items, using office supplies (i.e. scissors) and closing zip lock bags.
  • Fine manipulation for writing, handling paperwork and data entry on the computer.
  • Pushing and/or pulling force up to 30 pounds vertically, occasionally, and up to 10 pounds horizontally, frequently.
  • Lifting various ranges from the floor to overhead with weights up to 5-15 pounds frequently, from the floor to 48 inches with weights up to 30-40 pounds while stocking supplies occasionally, and from floor to waist with weights up to 15 pounds to handle reams of paper and files.
  • Employees working in this area may handle frozen products for several hours while assisting to organize groceries for each home.
  • Carrying up to 15 pounds various distances.
  • Step ladder climbing to access higher shelves for filing, stocking and/or retrieving.
  • There is frequent standing, sitting, and walking which are in bursts of 5 to 10 minutes. However, there are times when this can be extended to pacing with the consumer (time could be undetermined), and at times sitting and standing can be extended with the employee having the ability to change positions as permitted by the observance and behavior of the consumer at that time.
  • There is occasional forward bending from the hips while kneeling to reach to the side.
  • There is an occasional need to be able to assume a full squat/lunge from a standing position.
  • There is an occasional need to maintain back extension while being on stomach with one’s own body weight on hips and legs. This position is maintained for several minutes.
  • Occasionally, be able to put one’s own body weight on one knee and one elbow on the same side of one’s body.
  • Occasionally, be able to kneel and reach to same side toward the floor.
  • Lift up to 10 pounds from the floor to various levels, including from the floor to overhead occasionally during shift.
  • Have the ability to lift up to 50 pounds to assist in Activities of Daily Living, as required. Be able to lift up to 50 pounds from floor to waist.
  • Strong grasp is needed to use keys to open doors throughout the facility, and to maintain constant contact with a consumer during a restraint procedure or with other hold techniques.
  • Pushing/pulling force up to 25 pounds occasionally during shift.
  • There are times that require the employee to be able to ambulate as quickly as possible to assist coworkers with a consumer in a restraint procedure. (Example jog/run up to 0.1 mi).
  • At times activities may be inside or outside the facility; therefore, some activities will dictate physical requirements for sports or games.
  • Ability to team lift/transfer adult person with limited mobility
  • Any other duties as assigned by supervisor.


Minimum Qualifications

Bachelor’s degree with at least two years’ experience in a direct service functional area.

Must be 23 years of age or older. Must possess a valid Alabama Driver’s License. Must have proof of current liability insurance. QDDP certification in Alabama required

Effectively demonstrates the application of adult specific competencies as outlined below, to be attained within (6) six months of employment.

  • QDDP Certification in Alabama
  • Knowledge of governmental regulations and standards pertaining to services for persons with developmental disabilities.
  • Knowledge of intellectual disabilities.
  • Knowledge of available community resources
  • Knowledge of cultural diversity
  • Knowledge of behavior management, crisis intervention and interventions common to individuals with intellectual disabilities, as well as, to non-violent crisis intervention practice
  • Knowledge of educational methods appropriate to adults with intellectual disabilities.
  • Knowledge of Alabama’s Nurse Delegation Program (NDP)
  • Knowledge of all applicable community mental health program standards (DMH, JC, Medicaid)
  • Successful completion of Defensive Driving
  • Successful completion of MAC certification
  • Knowledge of the growth and development of consumers related to intellectual, emotional, spiritual, and physical aspects.
  • Knowledge of basic sign language.
  • Knowledge of basic data collection.
  • Knowledge of Person Center Planning, natural supports, and the philosophy of self-determination.
  • Knowledge of Rights of persons served and the complaint/grievance process.
  • Knowledge of Incident reporting—TherAp and RL-6.
