Job Title: Case Manager
The Assertive Community Treatment Team provides intensive support to individuals who face challenges in receiving treatment due to difficulties with illness and medication management, inadequate living skills, and a limited or no support system. Our team aims to improve their situation, enabling them to transition to less intensive care.
We offer Adult Outpatient Programs, which provide the least restrictive level of care, focusing on recovery, problem resolution, symptom reduction, stabilization, and prevention of higher-level care. Treatment varies based on presenting symptoms.
Responsibilities:
- Monitor consumers' well-being in mental health, physical health, living, learning, working, and social environments.
- Travel to consumers' residences, including homes, boarding homes, or community-based living arrangements.
Essential Functions:
- Link consumers to community resources.
- Assess consumers' needs and monitor progress.
- Secure placement in the least restrictive environment.
- Identify goals and plan services.
- Advocate for consumers to secure community resources.
- Evaluate consumers for discharge planning.
Physical Requirements:
- Frequent standing, sitting, and walking in bursts.
- Occasional forward bending, kneeling, squatting, and back extension.
- Lifting up to 10 pounds occasionally.
- Maintaining strong grasp and pushing/pulling force up to 25 pounds occasionally.
Qualifications:
Minimum Qualifications:
Bachelor's degree in a mental health discipline and certification of case management and in-home intervention training by Alabama State Department of Mental Health within six months of employment.
Age 23 , valid verifiable driver's license, and clear driving record required.
