What are the responsibilities and job description for the Behavioral Health Case Manager/Qualified Professional position at Alternative Behavioral Solutions, Inc.?
This is full-time position non-salaried 40 hours per week, Monday-Friday, occasional weekends.
DEFINITION: The applicant must have a human service degree. The person in this position is responsible for complying with the organizational and department policies and procedure and to be involved in the reviewing and updating as PCPs, CFTs, Judicial Review documents, Intake packages, in addition to all federal required documents and managing consumers.
Communication Skills: Case Managers must be able to discuss medication, behaviors with the psychiatrists, therapists, medical physicians, parents, educators, DSS case managers and juvenile justice etc.
Major Responsibilities:
- To undertake any other duties, after negotiation, that may be considered appropriate by the Program and/or Clinical Director.
- To attend Case Management team meetings, activities and working Consumer group
- To use professional judgement in dealing with unplanned crisis/issues without direct supervision or senior management, which requires investigation, analyzing and decision making
- To make referrals and receive referrals to any appropriate provider to obtain support or resources
- To participate in the development and implementation of service changes within the program to individualized specific programs for Consumers
- To adhere to the Social Care policy and procedures
- To prepare Person Center Plans (PCP) for agencies
- To organize CFT meetings, produce the CFT notes within 24 hours of the meeting
- To be able to speak and disseminate information to any MCO regarding the Consumer
- Admits new clients by reviewing records and applications; conducting orientations.
- Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
- Maintains clients' records by reviewing case notes; logging events and progress.
- Prepares clients' discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
- Develop a detailed plan of action to meet these needs, set goals, and find necessary resources to meet the goals
- Maintain confidentiality, respect privacy, and preserve the clients’ routine and independence as much as possible
- Stay in touch with clients to ensure the services were beneficial and that their needs are still met after pointing clients in the
Qualifications
- Educated to Bachelor’s level or equivalent experience of professional development as a Qualified Professional
- Demonstrable experience of working in, or in close association with, MCOs in the community setting in a statutory or non-statutory organization.
Knowledge
- Knowledge of patients in a PRTF
- Knowledge of relevant policy and issues, particularly relating to community care.
