ACT is seeking a Program Manager for the Community Living Program. Our ideal candidate will have many responsibilities, like insuring that their assigned staff is providing excellent oversight and care of the individuals who live in their Independent Supported Living residences they are responsible for. ESSENTIAL FUNCTIONS OF THE JOB:
- Oversees all aspects of assigned Community Living homes ensuring effective communication, meeting community integration goals and achieving positive results for individuals in the program.
- Coordinate ongoing health care for individuals and ensure medical needs are met.
- Complete annual written assessment and participate in the Individual Planning process for each individual.
- Hire, train, supervise and evaluate the performance of direct care staff on an ongoing basis.
- Maintain a safe and clean environment for individuals and an organized work space for staff.
- Ensure the shift documentation, documentation records and event reporting are completed in a timely manner.
- Ensure timely completion of employee payroll.
- Maintain ongoing professional communication with internal and external stakeholders.
- Schedule staff coverage for homes, following established budget.
- Ensure the maintenance of homes and communicate with landlord as needed.
- Provide accurate written progress reports on a monthly basis.
- Coordinate with the accounting department to ensure financial stability of individuals.
- Coordinate with other agencies (DFS, SSA) to ensure there are no interruptions of services.
- Present in assigned homes on a weekly basis, providing support to staff and completing necessary paperwork.
- Remain informed of, and assist in assuring compliance with appropriate program standards (DMH, CARF).
ADDITIONAL JOB FUNCTIONS:
- Provide on-call coverage throughout the week and on a rotating schedule for weekends.
- Provide direct care coverage in the homes, if needed.
- Maintain certification in CPR, First Aid, NCI, Medication Administration, Abuse/Neglect Prevention, Positive Behavior Support, HIPAA, HCBS and any others required by the agency or supervisor. Accept responsibility to complete in-service prior to expiration date.
- Other duties as assigned.
