Responsibilities
- Monitor Alameda County and The Gathering Place (TGP) Programs
- Monitor and maintain all aspects of clients’ medical record (electronic and hard copy) charts including the creation of hard-copy files, identify missing documents, and appropriate communication and coordination with clinical supervisors and staff to obtain missing documents
- Complete monthly staff attestation
- Register new clients in AFS and County MIS; open/close/update treatment episodes in AFS and County MIS; enter service data into County MIS; file all client-related materials in a timely fashion
- Enter billing data into County MIS; reconcile data entry by comparing AFS and County MIS reports for accuracy
- Support structured file review activities (Clinical Quality Review Team [CQRT], Program Utilization Review Quality Committee [PURQC] and 60 Day Review) including gathering and preparing charts, reviewing charts for the presence/absence of documents, review findings and entering data of findings into Electronic Record
- Create, distribute, monitor, and follow-up on Quality Management reports regarding presence/absence of key chart documents [assessments and treatment plans, annual Client and Services Information [CSI] updates, Payer Financial Information [PFI], etc.) in compliance with County, State and Federal regulations
- Provide technical assistance and support to clinical supervisors and staff as needed to maintain charts by specified deadlines
- Ensure compliance with The Health Insurance Portability and Accountability Act (HIPAA) requirements
- Obtain and track clinical staff identification numbers (National Provider ID number; County-specific billing ID)
- Work effectively with County Billing Systems
- Data entry, data cleaning and monitoring
- Assist with compiling, inputting, and reviewing data
- Assist with reporting of children’s health and social services’ outcomes data
- Assist in identifying system problems and development issues, and assist in identifying data/evidenced-based solutions, procedures, and policies to improve practice and systems at AFS.
- Use different databases and software (Microsoft Office programs, Electronic Health Records, and County specific Database) to clean, compile and integrate datasets
- Consult and collaborate with program staff to ensure program compliance
- Make copies and files as needed, screen incoming calls for staff, assist office staff with office maintenance and phone and office coverage
- Assume other duties as assigned by supervisor pending program needs
- Travel to AFS and County offices for trainings and meetings as needed
Qualifications
- Undergraduate degree in a relevant area (e.g., social sciences, computer sciences, psychology, social work, pre-Med, pre-Pharmacy, etc.) or a high school diploma with a minimum of two years related experience and/or training in Quality Management activities (EPSDT documentation guidelines, quality assurance reviews, medical records maintenance, etc.) and/or Quality Improvement activities (data management/analysis/reporting)
- Intermediate to advanced skills in Windows OS, MS Excel, Outlook, and PowerPoint.
- Possess ability to work quickly while maintaining focus and concentration to complete tasks with minimal errors.
- Able to work independently
- Ability to coordinate projects as necessary
- Able to manage time effectively and seek supervision when problems are encountered.
- Interpersonal skills including openness to feedback, non-defensiveness communication, and supportive approach to team dynamic
- Able to listen and orally respond to clinical and non-clinical staff questions, needs and problems in an appropriate way.
- Able to communicate clearly in writing (e.g., e-mails) to clinical and non-clinical staff in an appropriate way.
- Able to address common inquiries or complaints from customers, regulatory agencies, or other collaborating agencies.
- California driver’s license and auto insurance showing as an insured driver on the policy.
- Obtain and maintain fingerprint and government background clearances
- Able to read and write fluently in English
