Position Location: Tyngsboro, MA 01879

Schedule: Monday-Thursday 12:00 PM- 8:00 PM and Sunday- 9:00 AM- 5:00 PM

(Days Off: Friday & Saturday; 40-hours per week)

Position Overview:

The support coordinator oversees a residential program for 5 adults with intellectual, developmental, and physical disabilities. As the Supports Coordinator, you will manage the operational and financial operations of the residential program site. In addition, provide supervision, training, and evaluation of direct care professionals assigned to work at that location to ensure person-centered programming for individuals served under the direction of the Residential Director.

Duties and Responsibilities (These duties are a general summary and not all inclusive):

  • Ensure policies and best practices are implemented to produce optimal treatment, health, advancement, and safety of individuals served.
  • Ensure that the home is safe, clean, well-organized, and maintained in accordance with all local state and federal regulations.
  • Ensure that employee's complete all assigned duties and that all documentation is completed appropriate and timely.
  • Provide on-the-job training.
  • Manage financial documentation within the residence.
  • Delegate specific areas of responsibility to employees.
  • Adhere to the Administration of Medication policies and procedures, including the ordering, receiving, storing, documenting, and administering of medications.
  • As the MAP Site Supervisior, conduct weekly MAP medication audits at the program site.
  • Support and participate with individuals in various daily activities and/or routines at home and in the community by promoting self-advocacy, decision-making, and independence (activities may include recreational events, appointments, shopping, exercising, etc.).
  • Promote and encourage active participation in community events. Transport individuals to various community activities (activities may include work, recreational activities, and other engagements as needed).
  • Assist individuals with activities of daily living and home maintenance and perform cleaning responsibilities (activities may include meal preparation, personal hygiene, housekeeping tasks, laundry, etc.). Guidance and role modeling should be provided as appropriate.
  • Support individuals by assisting in the implementation and data collection of their Individual Service Plan (ISP) objectives.
  • Complete daily shift documentation and data collection.
  • Support individuals with maintaining relationships with family members, and friends, and developing new relationships.
  • Under the direction and supervision of ASI's Clinical Services Department, follow and implement behavior support plans as written.
  • Maintain individuals' confidentiality and records; preserve individuals' human rights by complying with applicable laws and regulations.
  • Ensure the ongoing health and safety of the individuals we support.

Qualifications:

  • A Bachelor’s degree in a health/human service-related field in the human services field, with at least two years of experience in a supervisory role is strongly preferred. Educational requirements may be waived in lieu of 5 years of supervisory related experience.
  • Possess current and valid Class D Motor Vehicle Operator's License
  • Applicants must pass the Medication Administration Program (MAP) certification. (ASI will provide training)
  • Applicants must obtain and maintain Cardiopulmonary Resuscitation (CPR) and First Aid (FA) certifications. (ASI will provide training)
  • Applicants must obtain and maintain Medication Administration Program (MAP) Site Supervisior Certification (ASI will provide training)
  • Knowledge of all applicable DDS Regulations.
  • Proven leadership, organizational and planning skills.
  • Ability to teach and inspire teamwork and cooperation.
  • Excellent verbal and written communication skills.
  • Skilled at problem solving, developing solutions.
  • Proficient in Microsoft Office Suite.

Sign-on Bonus:

$500.00 Sign-on Bonus will be provided after 90-Days of employment.
