Your Role :
As a Referral Navigator, you'll be the first point of contact for individuals seeking our services. You'll build rapport, conduct initial screenings, and guide them towards the resources that best meet their needs. This office-based position primarily involves phone interactions, but occasional in-person meetings may be required.
What You'll Do :
- Intake Coordination : Provide information about our services, conduct biopsychosocial intakes, assess needs, and provide information about our programs and services.
- Referral Expertise : Develop a deep understanding of our offerings and community resources, ensuring clients are connected to the most appropriate level of care.
- Collaboration : Build strong relationships with internal teams, external agencies, and community partners to ensure seamless coordination of care.
- Community Engagement : Represent AWS at community events and meetings to strengthen our network and raise awareness.
- Data Management : Maintain accurate and organized records of referrals, intakes, and outcomes.
- Transitional Support : You will provide per diem support for a minimal number of clients upon assigning them to providers or during other times of transition in the BHH Program. For example, if intakes are on hold you may support the BHH program with various tasks.
- Passion for Helping Others : A genuine desire to make a positive impact in the lives of individuals and the community.
- Communication Skills : Outstanding interpersonal and communication skills to connect with diverse individuals.
- Organizational Skills : Strong organizational and time management abilities to handle multiple tasks efficiently.
- Preferred Experience : Prior experience in behavioral health or social services is a plus.
