Job Summary

This individual will oversee street outreach activities, coordinate support services, and ensure survivors receive comprehensive care tailored to their unique needs. The Crisis Case Planner will collaborate with community partners, advocate for survivor-centered approaches, and support the organization’s mission to empower vulnerable individuals.
Key Responsibilities
Case Management
- Provide client-centered case management for survivors of DV and HT, including intake, needs assessment, safety planning, and goal setting.
- Develop and monitor individualized service plans that address the immediate and long-term needs of clients.
- Facilitate access to resources, such as housing, healthcare, counseling, legal assistance, and employment training.
- Maintain accurate, confidential, and up-to-date case files and documentation in compliance with organizational and legal standards.
Crisis Intervention
- Respond to crisis calls, walk-ins, and referrals with sensitivity and urgency, ensuring immediate safety and support for clients.
- Assess risk levels and implement safety measures, including connection to emergency shelters, law enforcement, or protective services.
- Provide emotional support and stabilization during times of acute distress.
Street Outreach Management
- Oversee AFG’s street outreach program, ensuring outreach workers are trained, equipped, and supported to engage with vulnerable populations effectively.
- Coordinate outreach activities to identify individuals at risk of or currently experiencing DV or HT.
- Build rapport with at-risk individuals to provide education, resources, and connections to AFG services.
- Track and evaluate outreach efforts to improve engagement and outcomes.
Program Support
- Facilitate group sessions and workshops focused on DV and HT prevention, education, and recovery.
- Collaborate with the Human Trafficking and Domestic Violence Support teams to ensure seamless delivery of services.
- Serve as a liaison between AFG and external agencies, including law enforcement, healthcare providers, and social service organizations.
- Advocate for clients’ needs within multidisciplinary teams to ensure they receive holistic and equitable support.
Training and Advocacy
- Train and mentor outreach workers, volunteers, and interns on DV and HT dynamics, trauma-informed care, and best practices.
- Raise community awareness about DV and HT issues through presentations, workshops, and events.

- Bachelor’s degree in social work, psychology, counseling, or a related field preferred.
- Minimum of 3 years of experience in case management, crisis intervention, or a related role, particularly with survivors of DV and/or HT.
Knowledge & Skills
- Strong understanding of DV and HT dynamics, trauma-informed care, and survivor-centered approaches.
- Familiarity with community resources and systems for survivors, including legal, healthcare, and housing services.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work effectively under pressure and handle crises with professionalism and empathy.
- Proficiency in data management systems.
