Alternatives For Girls is seeking a licensed social worker to join their Early Childhood Education team.
The Family Wellness Specialist promotes the health and well-being of pregnant and parenting mothers and their children who are experiencing homelessness and other vulnerabilities. Services that are provided to this underserved population include comprehensive case management, prenatal care and childbirth preparation, postpartum and early childhood support, and advocacy.
Responsibilities include:
- Provide comprehensive case management for pregnant and/or parenting participants and their children in shelter, transitional living, and aftercare programs.
- Conduct intake assessments and develop individualized care plans to address maternal and infant health needs.
- Facilitate Medicaid-compliant group sessions on topics including pregnancy, childbirth, postpartum care, infant care, safe sleep, and developmental milestones, as well as foster peer support groups to help reduce social isolation and stress among young mothers.
- Conduct maternal depression screenings, substance use assessments and child development screenings as well as coordinate referrals for intervention services.
- Conduct home visits for up to 12 months post-discharge and connect mothers with essential services, including Women, Infants & Children (WIC), Supplemental Nutrition Assistance Program (SNAP), Medicaid, and childcare assistance.
- Attend weekly team case meetings, staff meetings, trainings, and professional development workshops as required, as well as maintain accurate and up to date Medicaid-compliant documentation.
Qualifications include:
- Master’s degree in Social Work, Public Health, Maternal & Child Health, or a related field.
- Licensed Social Worker (LMSW, LLMSW, or equivalent) required; must have or be able to obtain certification in childbirth education, post-partum support and perinatal mental health from MDHHS approved vendor.
- Experience working with pregnant/parenting women, young children, and individuals experiencing homelessness, trauma, or domestic violence.
- Willingness to maintain a flexible schedule including evening and weekend hours, if necessary, to meet the needs of the residential and non-residential participants.
- Valid Michigan driver’s license, good driving record and access to a personal vehicle.
- Must meet all agency licensing requirements, including TB test, DHS clearance, and background checks.
