Alternatives is a private, non-profit organization serving sixteen counties in Midwestern Illinois. Since 1984, our mission has been to promote the independence and quality of life for older adults, adults with disabilities and their families. Alternatives' professionals are connecting with ONE IN FIVE individuals over the age of 60 in a sixteen-county region.
Our Care Coordinators are tasked with visiting individual’s homes, aged 60 and older, and completing Comprehensive Care Coordination assessments and re-assessments to evaluate the individual’s strengths, current support, problem areas and service needs.
From those assessments and re-assessments, our Care Coordinators develop a plan of care to establish services that will continue to promote an individual’s independence and quality of life to the greatest extent possible.
Care Coordinators are also tasked with completing Choices for Care screenings for nursing home placement and evaluates the potential for diverting them into the community with supportive services in place.
Our Care Coordinators follow program compliance and quality assurance guidelines to ensure they are providing the best quality in an appropriate time frame. Care Coordinators can expect to develop a greater knowledge of resources, networking and handling emergency situations as they arise.

Job Requirements:
Minimum Qualifications: Preferred BA/BS degree in social science, social work or a related field or be an RN or BSN. One year of program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree. Alternately may be an LPN with one year of program experience which is defined as assessment of and provision of formal services for the elderly and/or authorizing service provision. This position requires regular travel in a designated area, so a valid driver’s license and automobile are required.
