We are searching for a CCS Adult Intake Care Coordinator!
Requirements
Applicants must have the ability to transport clients by having a valid Wisconsin driver's license and vehicle and auto insurance. Applicants must also have an Associate’s degree or Bachelor's degree (B. A. / B. S.) from a four-year college or university in human services; and one-year related experience and/or training; or equivalent combination of education and experience.
Job duties include:
- Receives referrals and coordinates admission of members into the Comprehensive Community Services program following the rules and regulations of DHS 36.*
- Initiates contact with the referred Comprehensive Community Services member and builds rapport using a strength-based, trauma-informed, recovery-oriented, and culturally sensitive approach.*
- Conduct client visits out in the community and/or in clients' homes. *
- Completes all intake forms, assessments, and screens with members and other identified service professionals to ensure member eligibility and appropriate program fit.*
- Educate the member on Comprehensive Community Services, their rights as individuals receiving services, and all other pertinent information to ensure member eligibility and appropriate program fit.*
- Ensures the member receives culturally, linguistically, and age-appropriate assessment services including service planning and initial service delivery in an appropriate and timely manner.*
- Facilitates all aspects of Comprehensive Community Services service planning as outlined in DHS 36.*
- Maintains confidentiality and privacy of member and family records.*
- Completes and submits all required or assigned documentation by due dates.*
- Attends all supervision, meetings, trainings, in-services, and events as directed.*
- Completes at least eight hours of ongoing training on an annual basis.*
- Attends Milwaukee County Comprehensive Community Services meetings as directed to develop the program and to discuss requirements. *
- Coordinates crisis services for the member to be provided by DHS 34 certified programs. *
- Participates in quality improvement projects. *
- Other duties as assigned. *
