Your Role as a Nurse Practitioner :

As a Primary Care Nurse Practitioner at Altheda, you will :

Deliver comprehensive care , managing acute and chronic conditions through thorough evaluations, treatment plans, and follow-ups.

Focus on preventive care , educating patients to proactively manage their health.

Conduct diagnostic testing and screenings to detect health issues early.

Collaborate with a multidisciplinary team to create personalized, evidence-based care plans.

Refer patients to specialists when necessary and ensure continuity of care.

Uphold regulatory standards and Altheda’s commitment to clinical excellence .

Engage patients and families as partners in achieving optimal health outcomes.

Qualifications :

Current Pennsylvania State License and DEA certification .

5 years of experience in primary care or family medicine (preferred).

A passion for patient education , preventive care , and holistic health .

Strong clinical judgment and a commitment to high-quality care .
