- Treatment Planning: Collaborate with the healthcare team to develop and implement individualized care plans for adult patients, addressing their specific needs and preferences.
- Medication Management: Administer medications and treatments as prescribed, ensuring accurate dosage and proper documentation.
- Monitoring: Regularly assess and record patients' conditions, reactions to medications, and progress, and report any changes or concerns to the healthcare team.
- Health Education: Educate patients and their families about their conditions, medications, and self-care techniques to promote optimal health and well-being.
- Documentation: Maintain accurate and complete medical records, ensuring compliance with all regulatory standards and protocols.
- Collaboration: Collaborate with physicians, registered nurses, and other healthcare professionals to ensure seamless coordination of care.

- Licensed Practical Nurse (LPN) License: Current and valid LPN license in state of practice.
- Education: Graduation from an accredited LPN program.
- Experience: Previous experience in memory care or a related field
- CPR Certification: Current Basic Life Support (BLS) and CPR certification.
- Interpersonal Skills: Strong communication and interpersonal skills to interact effectively with patients, families, and the healthcare team.
