Job Description

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in providing exceptional memory care services. As an LPN in our organization, you will play a crucial role in delivering quality healthcare to our adult patients, ensuring their comfort, safety, and well-being.

Responsibilities

• Patient Care: Provide skilled nursing care to adult patients, including assessment, medication administration, wound care, and monitoring vital signs.
• Treatment Planning: Collaborate with the healthcare team to develop and implement individualized care plans for adult patients, addressing their specific needs and preferences.
• Medication Management: Administer medications and treatments as prescribed, ensuring accurate dosage and proper documentation.
• Monitoring: Regularly assess and record patients' conditions, reactions to medications, and progress, and report any changes or concerns to the healthcare team.
• Health Education: Educate patients and their families about their conditions, medications, and self-care techniques to promote optimal health and well-being.
• Documentation: Maintain accurate and complete medical records, ensuring compliance with all regulatory standards and protocols.
• Collaboration: Collaborate with physicians, registered nurses, and other healthcare professionals to ensure seamless coordination of care.
• Safety: Adhere to infection control protocols, safety guidelines, and emergency procedures to maintain a safe healthcare environment.
• Comfort and Support: Provide emotional support and comfort to patients and their families during challenging times.

Qualifications:

• Licensed Practical Nurse (LPN) License: Current and valid LPN license in state of practice.
• Education: Graduation from an accredited LPN program.
• Experience: Previous experience in memory care or a related field
• CPR Certification: Current Basic Life Support (BLS) and CPR certification.
• Interpersonal Skills: Strong communication and interpersonal skills to interact effectively with patients, families, and the healthcare team.
