- Monitor and assess residents' vital signs and condition, reporting any changes promptly.
- Assist with resident admissions, transfers, and discharges, ensuring proper documentation and communication.
- Provide wound care, injections, and other medical procedures as needed.
- Collaborate with the interdisciplinary team to develop and update individualized care plans for residents.
- Assist in maintaining a clean and safe environment for residents and staff.
- Document all nursing care provided accurately and in a timely manner.
- Educate residents and their families on healthcare procedures and provide emotional support.
- Respond to emergencies and provide first aid when necessary.
- Adhere to all state and federal regulations, as well as facility policies and procedures.
- Participate in ongoing education and training to maintain licensure and stay current with best practices in nursing.
- Valid Licensed Practical Nurse (LPN) license in state of practice.
- CPR certification required.
- Previous experience in a skilled nursing facility or long-term care setting is preferred.
