Alto Health Care Staffing is seeking an experienced Medical Records Coordinator LPN to join our team. In this role, you will play a vital part in maintaining accurate and up-to-date medical records for our residents.
Responsibilities:
- Maintain and update medical records, including residents' charts and care plans.
- Monitor and assess residents' health status, documenting observations and changes in condition.
You will be responsible for providing direct nursing care to residents with various medical conditions and needs. This includes administering medications and collaborating with the interdisciplinary healthcare team to develop and implement individualized care plans.
Additionally, you will implement infection control measures to maintain a safe and healthy environment. You will report any significant changes in residents' conditions to the charge nurse or physician promptly.
Requirements:
- Minimum 1 year of nursing experience, preferably in long-term care.
- CPR and First Aid certification.
- Compassionate and patient-centered approach to care.
