Key Responsibilities

• Conduct comprehensive assessments of residents' health status, including physical and psychological needs, and develop individualized care plans.
• Administer medications and treatments as prescribed by physicians, and monitor residents' responses to ensure their well-being.
• Provide wound care and manage skin integrity, documenting any changes or concerns.
• Continuously monitor residents' vital signs, report any abnormalities, and take appropriate action as needed.
• Educate residents and their families about health conditions, treatment plans, and self-care techniques.
• Work closely with the interdisciplinary healthcare team to provide comprehensive care and support to residents.
• Maintain accurate and up-to-date medical records, documenting all care provided and any changes in residents' conditions.

Requirements

To be successful in this role, you will need:

• A minimum of 5 years of nursing experience, preferably in a skilled nursing facility or long-term care setting.
• Knowledge of state and federal regulations governing nursing care in long-term care facilities.
