Job Title: Licensed Practical Nurse (LPN)
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our interdisciplinary healthcare team in our long-term care facility. As an LPN, you will play a vital role in ensuring the well-being and comfort of our residents.
Patient Care Responsibilities:
- Provide high-quality nursing care to residents with various medical conditions and needs
- Monitor and assess residents' health status, documenting observations and changes in condition
- Implement infection control measures to maintain a safe and healthy environment
- Maintain accurate and up-to-date medical records, including residents' charts and care plans
Collaboration and Communication:
- Collaborate with registered nurses (RNs), certified nursing assistants (CNAs), and other healthcare professionals to develop and revise care plans
- Attend team meetings and provide input on resident care and treatment plans
- Communicate effectively with residents and their families, addressing their questions and concerns with empathy and professionalism
Safety and Compliance:
- Adhere to all applicable healthcare regulations, including HIPAA and state nursing guidelines
- Maintain a clean and organized work environment, following infection control protocols
- Participate in ongoing training and education to stay current with nursing practices and technology
Requirements:
- Minimum of 5 years of nursing experience, preferably in long-term care
- CPR and First Aid certification
- Compassionate and patient-centered approach to care
