Location: Altru Business Center 2401 Demers Ave Grand Forks, ND 58201 Pay Range: $37.26 - $55.89 Summary: Evaluates and treats patients suffering from a physical disability due to injury, disease, or surgery in a home care environment. Establishes treatment plans and treats patients using a variety of methods to ease patients' pain and help increase mobility. Works cooperatively with interdisciplinary teams. Monitors, documents, and reports patients' progress.Supervised by the Supervisor and Manager of Home Health. Essential Job Functions: Performs evaluations and develops an intervention plan according to the patients age, diagnosis, symptoms and physician orders and documents all information per chart and review standards. Orients patients, family and/or other appropriate parties to the purpose and procedures of the therapy evaluation and treatment. Provides appropriate patient education, family teaching and support which relates to the needs of the patient and family. Identifies significant changes in patient's condition and takes appropriate actions, as evidenced in reporting, staffing consultations and documentation. Coordinates treatment schedules with disciplines using appropriate scheduling procedures; participates with inter-disciplinary team in the development and accurate documentation of a discharge plan in the inpatient settings. Identifies patient’s appropriateness for discharge with communication with other interdisciplinary team members and completing discharge process, documentation and coordinating plans for continued care as needed post discharge. Provides supervision over physical therapist assistants and therapy aides in a collaborative manner following license standards. Performs pain assessment and takes appropriate action according to standard operating procedure. Performs other duties as assigned or needed to meet the needs of the department/organization.
