TheRegistered Nurse provides direct nursing care in accordance withestablished policies, procedures and protocols of the organization.The nurse maintains the knowledge and skills necessary to providecare appropriate to the age of the patients served on the assignedunit. The nurse facilitates the coordination of patient care withinthe interdisciplinary care team and provides patient education toassure safe patientdischarge.

Essential JobFunctions :

• Performs dailyrounds on all patients assuring quality customer service bycoordinating and taking action on any requirements notmet.
• Develops a plan of care based onassessment that reflects the patient's physical, psychological,social, educational, developmental and rehabilitative needs basedon patient's age, ethnic origin, economic status, lifestyle, creedor philosophical beliefs.
• Assessesthe patient's condition and nursing needs, sets goals, andprescribes appropriate nursing actions to meet those goals and thephysical, psychological, social, and rehabilitative needs of thepatient, including dischargeplanning.
• Assesses, intervenes, andreassesses the patient's self-report / symptoms of pain promptly andtakes appropriate action according to standard operatingprocedure.
• Identifies the earlywarning signs of a change in a patient's condition and respondsappropriately to a deteriorating patient, including contactingresponsible clinicians as indicated by standard operatingprocedure. Assists with or institutes emergency measures for suddenadverse developments in patientconditions.
• Reports pertinentobservations and reactions regarding patients to the appropriatemultidisciplinary team member and records those observationsaccurately and concisely. Ensures that intervention or referralsare made promptly. Completes and submits all incident reports(patient, visitor, staff) prior to the completion of eachshift.
• Establishes and maintainscommunication and utilizes educational opportunities with patientsand other significant individuals.
• Asearly as possible, begins discharge planning, appropriate educationand counseling based on identified needs to optimize patient / familyunderstanding. Evaluates and documents according to standardoperating procedure.
• Assigns ordelegates, within the scope of practice, education and experience,the responsibility for performance of nursing interventions toother persons for which that person has the necessary skills andcompetence to accomplishsafely.
• Performs other duties asassigned or needed to meet the needs of thedepartment / organization.
