Job Overview
Altru Health seeks a skilled Licensed Practical Nurse (LPN) to join our team. This role involves performing and documenting patient assessments, ensuring health history is completed, and providing patient care.
Key Responsibilities
- Assist in identifying patient needs, priorities of care, and goals based on age, ethnic origin, economic status, lifestyle, creed, or philosophical beliefs.
- Evaluate the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions.
- Demonstrate competency and ability to carry out department-specific functions according to standards of care, policies, and procedures.
- Function as a member of the multidisciplinary team, contributing to an integrated patient-centered care plan.
- Assign or delegate nursing interventions to other personnel with necessary skills and competence.
- Identify early warning signs of a change in patient condition and report appropriately.
- Implement nursing interventions and prescribed medical regimens in a timely and safe manner, documenting care provided accurately and timely.
- Collaborate and communicate relevant patient information with patients, families, and multidisciplinary team members to ensure quality and continuity of care.
Certifications and Requirements
- Licensed Practical Nurse certification from the Minnesota Board of Nursing prior to start date.
- Basic Life Support certification from the American Heart Association within 90 days of start date.
Work Environment
This role requires occasional sitting, frequent standing, walking, stooping/bending, reaching, and handling/finger/feel. The job demands medium weight lifting, carrying, pushing/pulling, and sliding/transfer. Work will be conducted primarily indoors with no outdoor or extreme temperature requirements.
