Job Description
The Licensed Practical Nurse (LPN) plays a vital role in delivering high-quality patient care. As a member of the healthcare team, this individual will perform and document assessments, ensure health history and admission processes are completed, and provide patient education.
Key Responsibilities:
- Conduct thorough patient assessments and document findings accurately.
- Collaborate with registered nurses and other licensed practitioners to identify patient needs and priorities.
- Develop and implement individualized care plans that address patient-specific goals and objectives.
- Provide direct patient care, including administering medications, performing personal cares, and implementing interventions as prescribed by licensed practitioners.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
Requirements:
- Current licensure as a Licensed Practical Nurse in the state of Minnesota.
- Basic Life Support (BLS) certification within 90 days of hire date.
- Ability to work collaboratively as part of a multidisciplinary team.
- Excellent communication and interpersonal skills.
- Strong problem-solving and critical thinking abilities.
