Pay Range : $20.75 - $31.12
Summary : We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Behavioral Health team. In this vital role, you will perform and document assessments while ensuring comprehensive health histories and facilitating the admission process. You will play a key role in identifying patient needs, providing care, and coordinating discharge planning and education.
Essential Job Functions :
- Assist registered nurses in identifying patient needs and establishing care priorities tailored to individual circumstances such as age, ethnicity, and lifestyle.
- Evaluate care plans by gathering, observing, and communicating responses to nursing interventions.
- Work collaboratively with a multidisciplinary team to implement a patient-centered care plan.
- Delegate responsibilities for nursing interventions to appropriately skilled team members.
- Identify early warning signs of changes in patient conditions and implement interventions accordingly.
- Communicate relevant patient information to families and team members to ensure continuity of care.
- Promptly report patient symptoms of pain and collaborate with healthcare providers for effective management.
- Contribute to health education initiatives approved by licensed practitioners.
- Assist with discharge planning, education, and counseling to empower patients and families.
Certification Requirements :
Note : Temporary LPN licenses accepted prior to start; must hold an unencumbered nursing license before beginning work.
