Responsibilities and Duties
- Assess patients' needs, prioritize care, and develop effective plans of action in collaboration with registered nurses and other licensed practitioners.
- Contribute to the evaluation of patient care by gathering data, observing patient responses, recording information, and communicating effectively with the healthcare team.
- Modify plans of care as needed, working closely with licensed practitioners to ensure optimal patient outcomes.
- Perform department-specific functions, adhering to standards of care, policies, and procedures, while also completing additional duties as assigned.
- Function as part of a multidisciplinary team, implementing integrated patient-centered care plans and contributing to quality and continuity of care.
- Delegate nursing interventions to others when appropriate, ensuring they have the necessary skills and competence to accomplish tasks safely.
- Identify early warning signs of changes in patient condition, reporting promptly and implementing timely and safe nursing interventions.
- Document nursing care provided accurately and on time, ensuring clear communication with patients, families, and other healthcare team members.
- Participate in health education approved by licensed practitioners, assisting in discharge planning, education, and counseling to optimize patient understanding.
