Job Summary
The Licensed Practical Nurse (LPN) plays a vital role in our healthcare team, providing direct patient care and contributing to the development of personalized treatment plans. As an LPN at Altru Health, you will have the opportunity to make a meaningful difference in the lives of our patients.
Key Responsibilities
- Assist registered nurses and other licensed practitioners in identifying patient needs, priorities of care, and goals based on patient characteristics.
- Contribute to the evaluation of care plans by gathering, observing, recording, and communicating patient responses to nursing interventions.
- Demonstrate competency and ability to carry out department-specific functions according to standards of care, policies, and procedures.
- Function as a member of the multidisciplinary team, contributing to the implementation of integrated patient-centered care plans.
- Assign or delegate responsibility for performance of nursing interventions to others with necessary skills and competence.
- Identify early warning signs of changes in patient condition and report accordingly.
- Implement nursing interventions and prescribed medical regimens in a timely and safe manner, documenting nursing care provided accurately and promptly.
- Collaborate and communicate relevant patient information with patients, families, and other team members to ensure quality and continuity of care.
- Report patient self-reported symptoms of pain promptly according to standard operating procedure.
- Participate in health education approved by licensed practitioners.
- Assist in discharge planning, education, and counseling based on identified needs to optimize patient understanding.
Required Skills and Qualifications
- Licensed Practical Nurse (LPN) certification from the North Dakota Board of Nursing or Compact State Agreement prior to start date.
- Minnesota Board of Nursing license within 12 months of start date.
- Driver's License with acceptable driving record.
- Basic Life Support (BLS) certification from the American Heart Association within 90 days of start date.
