As a Medical Care Specialist at Altru Health, you will play a vital role in delivering exceptional patient care. Key Responsibilities:
- You will collaborate with registered nurses and other healthcare professionals to identify patient needs, prioritize care, and develop individualized plans of care based on patient demographics, lifestyle, and beliefs.
- You will contribute to the evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to medical interventions.
- You will demonstrate expertise in department-specific functions, adhering to established standards of care, policies, and procedures.
- You will work closely with the multidisciplinary team to implement an integrated patient-centered care plan.
- You will assign or delegate medical interventions to other qualified individuals.
- You will recognize early warning signs of patient condition changes and report them accordingly.
- You will implement medical interventions and prescribed treatment plans in a timely and safe manner.
- You will communicate relevant patient information with patients, families, and healthcare team members to ensure quality and continuity of care.
- You will participate in health education approved by licensed practitioners.
- You will assist in discharge planning, education, and counseling based on identified patient needs.
Certifications:
- You must be a Licensed Practical Nurse (LPN) certified by the Minnesota Board of Nursing prior to your start date.
- You must obtain Basic Life Support (BLS) certification from the American Heart Association within 90 days of your start date.
Requirements:
- A temporary LPN license may be accepted prior to your start date.
- An unencumbered nursing license is required prior to your start date.
