**Job Summary**
As a Licensed Practical Nurse (LPN) at Altru Clinic, you will perform and document assessments appropriate to licensure, ensuring health history and admission processes are completed, and interventions or referrals are made when necessary.
The LPN identifies discharge planning and education needs, provides patient care, and performs personal cares. You will assist the registered nurse or other licensed practitioner in identifying patient needs, priorities of care, and goals based on patients' age, ethnic origin, economic status, lifestyle, creed, or philosophical beliefs.
You will contribute to the evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions. In collaboration with a licensed practitioner, you will modify the plan of care as needed.
Key Responsibilities:

- Licensed Practical Nurse | Minnesota Board of Nursing | Prior to Start Date
- Temporary LPN license accepted prior to start
