Patient Care Coordinator

Duties and Responsibilities

• You will assist in the discharge planning process, providing education and counseling to patients and families as needed.
• You will participate in health education approved by licensed practitioners.
• You will collaborate and communicate relevant patient information with patients, families, and multidisciplinary team members to ensure quality and continuity of care.
• You will report patient self-report/symptoms of pain promptly according to standard operating procedure.

Working Conditions

This role requires occasional driving, which may involve transporting patients or equipment. You will also spend time working in indoor environments with moderate temperatures.
