Job Description
The Patient Care Specialist plays a vital role in the healthcare team, collaborating with an interdisciplinary group to provide comprehensive care to inpatients and rehabilitation patients. This position requires independent judgment in planning and organizing patient plans of care, prioritizing their needs and goals.
Patient Interaction
- Orient patients and family members to therapy evaluations and treatments, providing tailored education and support.
- Select and perform evaluation and treatment methods suitable for each patient's age, diagnosis, symptoms, and physician orders, documenting as per chart review standards.
- Carefully document pertinent patient information, including precautions, orders, and initial plans of care.
Treatment Coordination
- Coordinate treatment schedules with disciplines using appropriate scheduling procedures.
- Participate in developing accurate discharge plans in inpatient settings, considering patient-specific needs and circumstances.
Program Development and Monitoring
- Monitor program effectiveness and outcomes, suggesting improvements or modifications as needed.
- Perform pain assessments and take action according to standard operating procedure.
Certifications and Requirements
- Physical Therapist certification from the North Dakota Board Of Physical Therapy prior to start date.
- Valid Driver's License with an acceptable driving record.
- BLS certification from the American Heart Association within 90 days of start date.
