Key Responsibilities:
- Evaluate and develop intervention plans that align with the patient's plan of care, utilizing evidence-based methods for remediation, restoration, compensation, adaptation, disability prevention, and health promotion.
- Deliver high-quality interventions, assess goal progress, and modify approaches as needed in collaboration with the therapy team.
- Orient patients, families, and relevant parties to physical therapy evaluation and treatment procedures, providing targeted education and training under the supervision of a licensed therapist.
- Provide guidance and oversight to therapy assistants and aides in a collaborative manner.
- Develop and document comprehensive discharge plans when patients have achieved their goals or reached maximum benefit from therapy, offering follow-up resources and support as necessary.
- Gather and document essential patient information, including precautions, orders, and initial plans of care, ensuring compliance with insurance guidelines and regulations.
- Meet third-party payer requirements by maintaining accurate documentation within established window periods and adhering to relevant insurance standards.
- Conduct pain assessments and implement appropriate actions according to standard operating procedures.
- Pursue other duties as assigned or required to meet departmental needs.
