The Practical Nursing Professional plays a crucial role in delivering high-quality patient care at Altru Health. Key responsibilities include performing thorough assessments, ensuring accurate health histories and admissions, and providing timely interventions or referrals as necessary.
Key Responsibilities:
- Collaborate with registered nurses and other licensed practitioners to identify patient needs, prioritize care, and set goals based on patient demographics, lifestyle, and beliefs.
- Contribute to the evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions.
- Demonstrate competence in department-specific functions, adhering to standards of care, policies, and procedures.
- Work as a member of the multidisciplinary team to implement an integrated patient-centered care plan.
- Assign or delegate nursing interventions to other qualified individuals.
- Identify early warning signs of patient condition changes and report accordingly.
- Implement nursing interventions and prescribed medical regimens in a timely and safe manner.
- Communicate relevant patient information with patients, families, and multidisciplinary team members to ensure quality and continuity of care.
- Participate in health education approved by licensed practitioners.
- Assist in discharge planning, education, and counseling based on identified patient needs.
Certifications:
- Licensed Practical Nurse, Minnesota Board of Nursing, prior to start date.
- Basic Life Support (BLS), American Heart Association, within 90 days of start date.
Requirements:
- Temporary LPN license accepted prior to start date.
- Unencumbered nursing license required prior to start date.
