The ideal candidate will maintain knowledge of psychiatric medications and identify significant changes or high-risk situations.
Key Responsibilities:
- Develops treatment plans and reviews assessment information with each patient. Assesses the patient to identify needs, issues, care goals, termination, and continuing care options.
- Completes psychosocial assessments, diagnostic impressions, counseling, and referrals for individuals, couples, families, or groups using various therapeutic approaches. Identifies significant changes or high-risk situations. Processes progress notes. Completes all billing procedures.
- Makes contact with referral sources, providers, and other caregivers to ensure communication and coordination exist regarding patient care. Collaborates with community resources to deliver services that promote quality outcomes and continuity of care.
- Monitors program effectiveness by evaluating, initiating, and participating in restructuring current and new programs.
- Performs utilization review when required.
- Collaborates with Primary Care Providers regarding patients' progress and changing needs.
- Performs other duties as assigned or needed to meet departmental/organizational needs.
- Licensed Clinical Social Worker (LCSW) from the North Dakota Board of Social Work Examiners prior to start date.
- Licensed Independent Clinical Social Worker from the Minnesota Board of Social Work within 12 months of start date.
- Licensed Professional Clinical Counselor (LPCC) from the North Dakota Board of Counselor Examiners prior to start date.
- Licensed Professional Clinical Counselor (LPCC) from the Minnesota Board of Behavioral Health and Therapy within 12 months of start date.
- Basic Life Support (BLS) certification from the American Heart Association within 90 days of start date.
- Candidates can be licensed as LCSW or LPCC.
- The position will be credentialed through Altru Health System's credentialing process.
