Job Summary
Altruistic Leadership, LLC is looking for compassionate, organized, and dedicated individuals to join our team as Coordinated Family Support Professionals. In this role, you will work with adults aged 18 and over with Intellectual and Developmental abilities (IDD) who live with their families. You will provide vital support by identifying service gaps in the home, helping families access community resources, and connecting them with culturally and linguistically appropriate services. This position requires excellent communication, initiative, and problem-solving skills, along with the ability to collaborate effectively with service providers, healthcare professionals, and community organizations to improve service delivery. We serve communities in the South Central Los Angeles area, including Bell, Huntington Park, East Los Angeles, Lynwood, South Gate, Compton, Downey, and surrounding cities under the catchment areas of South Central Los Angeles Regional Center (SCLARC) and East Los Angeles Regional Center (ELARC).
Duties
- Act as a liaison between families and service providers, ensuring the household unit receives the support and generic resources they need.
- Collaborate with professionals to develop and implement comprehensive care plans.
- Coordinate and monitor the delivery of services to ensure they meet both the needs of the individual and the family’s goals.
- Identify and connect families with a variety of social services, medical, housing, educational, and financial supports, facilitating referrals and follow-ups to ensure continuity of care.
- Provide education, training, and linkage to families of adults with IDD, offering information on coping strategies, behavioral support, and available community resources.
- Provide crisis intervention and short-term support to families in need, developing strategies to manage and resolve challenges that may arise in the home or community setting.
- Advocate for the rights and needs of families and individuals with IDD, ensuring they are informed about available resources and services that promote independence and integration into the community.
- Facilitate transportation, applications, and advocacy to local, state, and federal programs and agencies designed to support individuals and families.
- Work closely with case managers, social workers, healthcare providers, and other professionals to ensure coordinated and seamless care.
- Promote independence and self-determination for individuals with IDD, while ensuring families receive the necessary support and training to maintain a stable home environment.
- Maintain up-to-date records and documentation of family progress, services provided, and any concerns or issues that arise.
- Ensure a strengths-based approach to working with families and individuals with IDD, fostering resilience and long-term success.
- Ensure all services comply with HIPAA regulations.
- Follow and understand mandated reporting procedures and protocols.
- Participate in monthly trainings and adhere to Altruistic Leadership’s policies, procedures, and mission.
- Provide culturally and linguistically appropriate information and support.
- Perform other duties and support as assigned.
Requirements
- A minimum of Associates-level degree in Social Work, Human Services, or a related field of study or 3 years of experience in the developmental service delivery system.
- Be at least 18 years of age at the time of application.
- Reliable and safe transportation
- Valid California Driver's license, vehicle registration, and insurance (transportation is provided as part of the job duties, on a case by case basis).
- Driving record clearance (in good standing)
- Be able to clear Department of Justice background check
- Current First Aid & CPR certification
- TB Clearance
- Ability to lift a minimum of 25 pounds.
- Ability to handle sensitive information with confidentiality.
Job Types: Full-time, Part-time
Pay: $23.00 - $26.00 per hour
