Specialty Seymour CTC-AP-Physician-0015 State IN Location 357 Tanger Blvd Seymour Indiana 47272 Requirements Acadia- Seymour CTC Outpatient Addiction Medicine Clinic for 3 / 17-3 / 18 from 6am-12pm Max rate is $240 per hour Dates needed : -3 / 17-3 / 18 from 6am-12pm -Do you provide methodone or suboxone assisted treatments? Yes, both. -Does the locum need to have a Suboxone x waiver? Yes but I think the State of Indiana has done away with that requirement. But if possible let's stay with yes to be safe. -How many patients per day will the locum see? Mainly the Intakes / Admissions then maybe a few random patients here and there. No more than 12 I would guess. -Who does History and Physicals? The MD but we will try not to schedule on these days. -Will there be any other doctors or mid-levels working at the clinic with the locum? Unfortunately none. -What support staff is available? Primary support will be Nurse Supervisor and her Team. -What EMR do you use?
