Responsibilities
- Develop working relationships with internal sales team and facility staff to create excellent admission outcomes.
- Handles phone inquiries, checks for referrals, and processes them expediently and accordingly.
- Communicates with the Liaison and referral source, notifies them it has been received and an answer will be provided promptly on acceptance or denial of the referral.
- Reviews health records prior to accepting referral.
- Creates new patient digital charts.
- Verify primary insurance coverage and obtains authorization by calling insurance provider.
- Costs out medications and obtains approval from the Administrator, once approved notifies Liaison to complete referral evaluation.
Qualifications
- Minimum of one (1) year experience with admissions in a Skilled Nursing Facility, hospital discharge, or case management required.
- High School graduate/ GED required.
- Type 40 words per minute.
- Knowledgeable of medical practices, terminology and procedures, as well as laws, regulations and guidelines that pertain to home health.
- Able to relate positively, favorably and cooperatively with others, including physicians, associates, and personnel of outside agencies and organizations.
- Must be capable of working under pressure in a busy setting, projecting a friendly and professional image. Must be versatile and adapt to changing job duties and priorities, and/or emergent or stressful situations.
