What are the responsibilities and job description for the Per-diem RN for Homecare Agency position at Always An Angel Licensed Homecare?
Always An Angel Licensed Homecare is seeking Per-diem RN’s services with licensed homecare company serving Westchester, Putnam and Dutchess counties.
Provide Skilled Nursing services including:
- Medication Management
- Medication Pours
- Wound Care
- Document and Input all notes in CRM.
- Introduction and review of the patient care plan with HHAs/PCAs
- Compliance with all pertinent rules, regulations, standards, policies, and procedures
- Experience with and understanding of NYS DOH regulations and surveys (a plus)
- Conduct quarterly reviews with caregivers.
- Schedule follow up quality assurance visits as needed and six-month re-assessments.
- Adheres to all professional practice standards
- Use effective problem solving and decision-making strengths to rectify issues as they arise
- Assist with annual staff education and evaluation of personnel performance
- Must be flexible to work various hours and respond to emergencies
Qualifications:
- Current NYS RN or LPN license
- Recent physical (within the year)
- Current License (per facility contract or state regulation) required.
- Experienced and understanding of NYS DOH regulations and surveys.
- Current driver’s license and vehicle (There is mileage reimbursement)
- Computer knowledge
