Job Summary:

The Client Care Manager for Home Health Care will be responsible for coordinating, monitoring, and evaluating patient care services in the home health setting. This role includes collaborating with patients, families, caregivers, and healthcare professionals to ensure the delivery of quality care while meeting the unique needs of each patient.

Key Responsibilities:

  • Patient Care Coordination: Develop, implement, and update individualized care plans based on patient assessments, and ensure that appropriate services are provided in the home setting.
  • Collaboration: Work with physicians, nurses, therapists, and other healthcare providers to ensure comprehensive care for patients. Serve as the point of contact for the patient and their family.
  • Patient Advocacy: Advocate for patients by ensuring that their care needs are met and any concerns are addressed promptly.
  • Monitoring and Assessment: Conduct home visits as needed to monitor patient progress, assess care requirements, and evaluate the effectiveness of the care plan.
  • Documentation: Maintain accurate and up-to-date patient records, including assessments, care plans, progress notes, and discharge plans, in compliance with regulations and agency policies.
  • Education: Educate patients, families, and caregivers about health conditions, treatment options, and self-care techniques to promote optimal recovery and well-being.
  • Compliance: Ensure adherence to healthcare regulations, standards of practice, and organizational policies. Report any concerns or deviations from care standards.
  • Resource Coordination: Identify and arrange for community resources, support services, and equipment necessary for patient care.
  • Discharge Planning: Assist with discharge planning, ensuring that patients are prepared for a safe transition to other levels of care or home-based self-care.

Qualifications:

  • Education:
  • Bachelor’s Degree in Nursing, Social Work, or a related healthcare field

Experience:

  • Minimum of 2-3 years of experience in home health care, case management, or a related field.
  • Experience with care planning, patient assessments, and interdisciplinary collaboration is highly preferred.
  • Certifications:
  • Case Management Certification (CCM) preferred.
  • CPR and First Aid certifications as required.

Skills and Competencies:

  • Strong knowledge of home health care practices, policies, and regulations.
  • Excellent communication and interpersonal skills to interact effectively with patients, families, and multidisciplinary healthcare teams.
  • Ability to prioritize and manage multiple cases efficiently in a fast-paced environment.
  • Strong organizational and problem-solving abilities.
  • Empathy and the ability to provide emotional support to patients and families.
  • Proficient with electronic health record (EHR) systems and other healthcare software.

Compensation and Benefits:

  • Company Car: A company vehicle will be provided for work-related travel and home visits.

Work Environment:

  • Primarily home-based visits to patient residences, one day a week office admin day
  • Flexibility in working hours, including occasional evenings or weekends as needed.
  • Occasional travel between patient homes, hospitals, and other healthcare facilities.

Job Type: Full-time

Pay: $58,000.00 - $60,000.00 per year
