We are looking for a PRN Med Social Worker to join our team PRN. We are a Home Health Care Agency out of Grafton, Ohio and our patient population ranges from Medicaid to VA to Medicare.
The HHCSW will help the individual and family to set specific goals and develop a plan of care to achieve them. They will also provide education and resources to help the individual and family cope with the physical, emotional, and social impact of the patient's illness or injury. The role of an HHCSW is to assess the individual and family's needs, connect them with community resources, and advocate for their rights. This may include helping the individual and family access financial assistance programs, providing counseling, and connecting the individual and family with support groups or other resources.
