Personal Care Aide
Job Summary:
The Personal Care Aide provides limited assistance with personal care tasks and home management tasks, support and companionship as delegated on the individualized client care plan.
Qualifications:
Must have experience and/or training in Personal Care Aide skills validated by the RN, no substantiated findings with North Carolina Health Care Personnel Registry (HCPR), Criminal background check completed. Negative TB testing, Current and valid NC driver’s license and automobile insurance.
Physical Requirements:
Requires considerable physical effort most of the day. Requires squatting, kneeling, reaching, twisting, walking and sitting for long periods of time. Limited risk of exposure (see exposure control plan)
Mental Requirements:
Work involves a broad range of problems, many of a complex nature. Must be able to work independently, make sound judgments based on assessment and data available and act accordingly. Must be flexible, innovative, and possess good interpersonal skills. Must be able to prioritize multiple functions and/or tasks.
Reports to: Nursing Supervisor
Duties:
In-home services shall be provided in accordance with the client’s plan of care which is individualized to each client. In-home aides shall follow the instructions for client care as written by the Nursing Supervisor.
In-home aide duties may include, but are not limited to the following:
1. Provide limited assistance with personal care (ADL activities- i.e. bathing, care of mouth, skin, hygiene, toileting and continence, mobility, eating); and ADL activities.
2. Provide limited assistance with ambulation, assist feeding-special, take and record vital signs, record and empty catheter.
3. Assist client with self-administered medications which are ordered by a physician or other persons authorized by State law to prescribe.
4. Perform household services which are essential to the client’s care and delegated by the RN 5. Record and report any changes in the client’s condition, family situation or needs to the appropriate care coordinator
6. Limited assistance with prescribed exercises, which the client has been taught by appropriate professional personnel.
7. In-home aides must provide appropriate documentation of training and must complete competency checks prior to independently providing in-home aide services by the designated nurse.
8. In-home aides must complete each task as assigned for their client and document notes and concerns.
9. Participate in orientation and training as directed by the agency.
Other Duties:
1. Reports to work, as scheduled and is regular in attendance, arrives in a timely manner without excessive absences and follows procedures for proper notification of absences/tardiness.
2. Displays flexibility in scheduling and accepting assignments.
3. Reports to work with a clean, well-groomed appearance.
4. Promotes a desirable work environment among clients, coworkers - courteous and professional.
5. Conveys respect for staff and clients. Maintains a positive attitude at all times. Upholds client rights, including respect for property and confidentiality.
6. Upholds infection control principles and policies.
7. Completes other duties as assigned.
