Home Health Care LPN
Always Best Care Home Care is a fast-paced skilled home health care agency that is dedicated in providing skilled nursing and therapy services to our community and facility-based patients. We are searching for individuals to join our team. Always Best Care is searching for a licensed practical nurses with at least one year of experience as a licensed nurse. The territory includes Parma, Middleburg Heights, and Strongsville. This is a part-time or full-time position. PRN is not available.

Qualifications:
- Current unrestricted license in the state of Ohio
- BLS certification
- Minimum one year experience as a licensed nurse
- Valid driver’s license and reliable transportation
- Excellent customer service and communication skills
- The ability to work independently with an emphasis on time management
- Computer skills
- Knowledgeable and competent in wound care, foley catheter management, disease process management, and IV administration
Job Responsibilities:
- Provides direct skilled nursing care per Always Best Care policy.
- Completes thorough assessments and reports to the physician any changes in status to decrease the patient’s risk of re-hospitalization.
- Maintains open communication with all disciplines involved in the patients care.
- Completes documentation in a timely fashion in the electronic medical record.
