Job Overview
We are seeking a dedicated and compassionate HHA to provide personalized care to patients in their homes or other settings. The ideal candidate will have a strong background in nursing and a passion for delivering high-quality healthcare. This role requires the ability to work independently while ensuring the safety and well-being of patients with various medical needs.
Duties
- Deliver direct patient care assisting with daily living activities inside of client home.
- Develop and implement individualized care plans based on patient assessments and health goals.
- Provide specialized care for patients with specific needs, including those requiring infectious disease care, memory care, or support for developmental disabilities.
- Collaborate with family members and other healthcare professionals to ensure comprehensive in home patient support.
- Maintain accurate and up-to-date patient records, documenting changes in condition and response to treatment.
- Educate patients and their families on health management, medication administration, and preventive care strategies.
- Assist in coordinating follow-up appointments and referrals to other healthcare services as needed.
Experience
Candidates should possess the following experience:
- Previous nursing experience in settings such as hospitals, clinics, or outpatient facilities is preferred.
- Familiarity with home care practices and protocols is highly desirable.
- Experience working with pediatric patients or those with developmental disabilities is a plus.
