Private Duty Home Health Aide

Job Overview
We are seeking a dedicated and compassionate Private Duty CNA to provide personalized care to patients in their homes or other settings. The ideal candidate will have a strong background in nursing and a passion for delivering high-quality healthcare. This role requires the ability to work independently while ensuring the safety and well-being of patients with various medical needs.

Duties

• Deliver direct patient care assisting with daily living activities inside of client home.
• Develop and implement individualized care plans based on patient assessments and health goals.
• Provide specialized care for patients with specific needs, including those requiring infectious disease care, memory care, or support for developmental disabilities.
• Collaborate with family members and other healthcare professionals to ensure comprehensive in home patient support.
• Maintain accurate and up-to-date patient records, documenting changes in condition and response to treatment.
• Educate patients and their families on health management, medication administration, and preventive care strategies.
• Assist in coordinating follow-up appointments and referrals to other healthcare services as needed.

Experience
Candidates should possess the following experience:

• Previous nursing experience in settings such as hospitals, clinics, or outpatient facilities is preferred.
• Familiarity with home care practices and protocols is highly desirable.
• Experience working with pediatric patients or those with developmental disabilities is a plus.

Pay: $17.00 - $17.50 per hour
