Always Compassionate Home Care is seeking a Case Coordinator to work with our coordination team. The coordinator's primary responsibility is to work directly with our caregivers to ensure they are providing our clients with the compassionate care our clients deserve.
Coordinators focus on delivery of care through scheduling and continuous communication with client's and caregivers.
Qualifications :
Working knowledge of HHA Exchange
Excellent organizational and time-management skills
- At least 3-5 years of experience in a Home Care setting
- Great communication and interpersonal skills
- Strong customer service background
- Staffing open cases and providing daily support to clients and caregivers
Requirements :
- Staff personnel according to home care patient's schedules
- Documents and reports to Case Manager when there is a change in client status, change in service requirement, change in aide assigned to case, client, nurse incidents, and complaints.
- Ability to use time efficiently
- Ability to work in a fast-paced environment and demonstrated ability to multi-task
- Technically savvy - proficient in use of smartphones, tablets, laptops, and other devices
- Exceptional communication skills - demonstrated ability to communicate and follow-up in a timely manner
- Maintains a daily call log and monitors the call dashboard
- Communicate all relevant information to client family and client service coordinators as needed
- Follows up with clients, and families regarding schedule changes, service ues, etc.
- On call after hours required (Weekdays and Weekend)
