JOB SUMMARY
Under the supervision of the Case Management Director, the licensed LVN/RN Inpatient Case Manager will be responsible to provide Case Management services to members identified in an acute care setting or SNF. Inpatient licensed LVN/RN Case Manager follow these identified members through the continuum of care until deemed appropriate for discharge from acute care or SNF services.
QUALIFICATIONS

· Communicate professionally with all levels of health care physicians and hospital staff
· Respond to common inquiries from members, vendors, regulatory agencies, health plans, and members of the business community with excellent customer service skills.
· Ability to perform clinical nursing skills including assessment, planning, evaluations, and outcomes.
· Ability to apply the nursing process.
· Problem-solve, negotiate and demonstrate independent decision-making
· Prioritize with excellent organizational skills, responding to multiple demands and timeliness
· Demonstrate professional written and verbal communication skills and presentation skills using the English language
· Demonstrate ability to respond to common inquiries from patients, customers, vendors, regulatory agencies, health plans or member of the business community and possess excellent customer service skills
· Twist, turn and utilize reaching motion, ability to sit and/or stand for extended periods of time
· Able to work independently.
· Has knowledge of industry standards and expectations.
· Has knowledge of commonly used concepts, practices and procedures within this field.
· Excellent verbal and written skills.
Computer literate with knowledge of Excel/Word.
Responsibilities
- Provide case management services to patients, including coordinating care, conducting patient assessments, and developing individualized care plans.
- Utilize nursing expertise to ensure quality patient care and adherence to medical protocols.
- Collaborate with healthcare professionals to facilitate patient transitions and discharge planning.
- Apply knowledge of medical terminology, physiology, and HIPAA regulations in daily practice.
- Demonstrate strong communication skills to interact effectively with patients, families, and interdisciplinary teams.
- Utilize computer skills to document patient information accurately and efficiently.
Job Type: Full-time
Pay: $55,000.00 - $90,000.00 per year
