The Care Coordinator is responsible for partnering and collaborating with the agency’s scheduling coordinator, nurse and owners to ensure the client’s needs are met and exceeding the client’s expectations. A critical component of this role is to demonstrate the agency’s culture of compassion, accountability, and results by displaying the highest standards of integrity, superb follow-through, consistency, dependability, reliability, excellent customer service, and exceptional client satisfaction. QUALIFICATIONS

* At least seven years of professional caregiving experience

* Demonstrates self-motivation, flexibility, and the ability to adjust in the moment

* Experience working in a high stress environment with multiple demands

* Excellent written and verbal communication skills

* Experience working with clients in a professional manner

* Detail-focused and able to stay organized

* Proactive when getting assignments done with a sense of urgency

* Strategic thinking and problem-solving skills

REQUIREMENTS

* CNA license preferred but not required

* 7 years of caregiving experience

* Ability to pass background check and drug screen

* Experience with transfers/gait belts

* Experience with bed-bound preferred

* Ability to lift 45 pounds

* Ability to care for clients with both cats and dogs

* Because of the delicate nature of our clients we have a strict NO SMOKING policy

CARE COORDINATOR RESPONSIBILITIES

* Demonstrates support for the agency’s mission and vision statements.

* Primary Responsibility: Be available as a fill-in caregiver for all call-offs Monday through Friday from the hours of 7am to 11pm (first and second shifts).

* The expectations for being on call include but are not limited to: keeping your cell phone on so if AMADA’s owners, scheduler or nurse call or text, it is required that you answer and respond within 30 minutes and then be at a client's location within one hour to fill in. This is across AMADA Charlotte’s entire geographical footprint of clients being served. Agency provides company car.

* On occasion, the Care Coordinator could be asked to fill-in for a weekend shift if the agency is not able to find any other caregiver to care for the client. If this happens, all weekend hours will be paid as over-time to the Care Coordinator.
* Secondary Responsibilities:

* Attend AMADA Charlotte’s staff meetings if NOT asked to fill-in as a caregiver on that day.

* Attend and support AMADA’s training and orientations if NOT asked to fill-in as a caregiver on that day.

* Collaborate and coordinate with the agency’s nurse to ensure the care of Amada’s clients is exceptional.

* Provide verbal and written feedback to the agency’s nurse regarding client care and care plans as needed.

* Provide verbal and written feedback to the agency’s nurse regarding the agency’s caregivers while observing them with a client.

* Coach, counsel and encourage the agency’s caregivers under the supervision of the agency’s nurse.

* Assist and partner with the agency’s owners to help resolve client challenges and problems.

* Assist and partner with the agency’s owners to accomplish client tasks (i.e. insurance needs).

* Maintain confidentiality of client, personnel, and agency operation.

*CAREGIVER RESPONSIBILITIES *

* Provide personal hygiene assistance for elderly clients by attending to their basic personal care needs, such as showering, bathing, dressing, transferring and feeding

* Remind clients take prescribed medication and assist clients with ambulation

* Help with mobility and transportation (doctor’s appointments, walks etc.)

* Plan and prepare meals and help with physical therapy exercises

* Accompany the clients on shopping and grocery trips

* Perform housekeeping duties and report any unusual incidents

* Act quickly and responsibly in cases of emergency
