Licensed Vocational Nurse (LVN) – Non-Medical Home Care Case Manager
Location: North Dallas-Fort Worth Area

About the Role:
We are seeking a driven, highly organized Licensed Vocational Nurse (LVN) to join our growing team in a non-medical home care setting. This is not your typical LVN position—this role combines clinical insight with case management and project coordination to support our aging population in maintaining quality of life at home. The ideal candidate thrives in a fast-paced environment, is comfortable managing a broad client load, and has the confidence and autonomy to make things happen with minimal supervision.
Key Responsibilities:
- Provide case management oversight in collaboration with Registered Nurse for multiple non-medical home care clients.
- Conduct in-home visits and assessments throughout the North DFW region.
- Develop, implement, and monitor personalized care plans in collaboration with Registered Nurse, caregivers, clients, and families.
- Serve as the main point of contact for client needs, coordinating services and follow-up.
- Communicate effectively across teams and with external partners to ensure care continuity.
- Maintain accurate documentation and timely reporting.
- Lead with initiative and independence, owning your caseload and schedule.
- Contribute to project management initiatives aimed at improving operational efficiency and client outcomes.
- Prioritize and manage multiple responsibilities without losing attention to detail.
Qualifications:
- Active LVN license in the state of Texas.
- 1–5 years experience working with the geriatric population, preferably in home care, hospice, or case management.
- Strong case management experience required.
- Proven ability to multi-task and work independently under limited supervision.
- Excellent written and verbal communication skills.
- Comfortable with technology, mobile documentation, and CRM systems.
- Reliable transportation and willingness to travel throughout North DFW (mileage reimbursement provided).
- Experience with veterans or VA systems strongly preferred.
- Project management experience is a plus.
- Military service is a plus.
